Leaving a Legacy of Hope for Male Fertility with Dr. Ramy Ghayda

    2026-03-24 | 20 mins.
In this episode, I sit down with Dr. Ramy Ghayda, an experienced urologist and the Chief Medical Officer at Legacy, to dive deep into the world of male fertility. Dr. Ghayda brings his expertise in urology and male infertility to discuss why it's "always nice to have sperm on ice,"a phrase I say often because preserving male fertility is just as important as egg freezing. We explore Legacy's revolutionary approach to men's reproductive health, a digital fertility clinic that provides everything from at-home semen analysis and sperm freezing to DNA fragmentation testing, telehealth consultations, and nutritional support: all designed to fill the gap in fertility care that has traditionally focused only on women.

Throughout our conversation, Dr. Ghayda and I discuss the alarming decline in male fertility rates over the past few decades and what's causing it; from environmental toxins and endocrine-disrupting chemicals to lifestyle factors like diet, stress, and sleep. We talk about why fertility should be considered the sixth vital sign and how infertile men face higher risks of chronic diseases later in life. Dr. Ghayda also shares his passion for breaking the taboos around male infertility, rebalancing the emotional workload in couples struggling to conceive, and helping men understand that they are 50% of the fertility equation. This episode is a must-listen for anyone interested in optimizing their reproductive health or supporting their partner through the fertility journey.

    In this episode, we cover:

Legacy's comprehensive approach to male fertility, including at-home semen analysis, sperm freezing, DNA fragmentation testing, and telehealth support
The science behind declining sperm counts and quality over the past 50 years and what environmental factors may be contributing
How lifestyle choices (smoking, alcohol, marijuana, heat exposure, diet, exercise, and sleep) directly impact sperm production
Why DNA fragmentation testing matters and how it can reveal correctable fertility problems
The emotional toll of infertility on men and how to rebalance the conversation and workload between partners
Why fertility is now being considered the sixth vital sign and its connection to overall health and longevity
Practical steps men can take to be proactive about their reproductive health and support their partners

    Does Sperm Really Matter?

    2026-03-23 | 13 mins.
Does sperm really matter? If you tuned into the conversation with Dr. Paul Turek and I earlier this week, you'll know that of course it matters!

It truly does take both sperm and an egg to make a baby. Today I'll review the sperm question a bit, and explain why it matters. AND, I'll give you a lot more info about how to improve sperm quality.

Here's the thing: I know that guys want to hear what they want to hear. They have selective listening. I'm not kidding when I tell you that I had someone email me recently and say "My husband's sperm tested out GREAT, and when we went over the results with the doctor we heard two different things. He thinks he heard our doctor say that it means he can just keep drinking, smoking, and enjoying marijuana and doesn't need to make any lifestyle changes. I heard something very different, which was that he should still cut back until we are pregnant. Can you help settle this for us?"

And while it's an unpopular answer (at least with the guys), my response is: Men should cut back or abstain from drinking, and smoking (marijuana too) if you're TTC.

Here are a few of the things I suggest to anyone bringing the sperm side of the equation to the baby making situation:
    - consider freezing your own sperm for future use when you are young.
    - get in the best shape of your life if you're trying to conceive. Sperm matters, and men's health is directly related to their sperm quality.
    - take some prenatal vitamins. Yes, really. Many of the same supplements that I recommend for egg quality also are great for sperm quality.
    - cut back on smoking, drinking, and drug use. Marijuana negatively impacts sperm.
    - don't cook your balls: avoid hot tubs, using your laptop on your lap, and other situations that may create high temps for the area.
    - find ways to cut back on stress, if you live a stressful life. Mindfulness and meditation are great for everyone.

The Most Important Thing to Know Before IVF with guest Dr. Max Ezzati

    2026-03-19 | 43 mins.
Preparing for IVF can be an emotional and complex journey, and having the right information beforehand can make all the difference. In this episode, I'm joined by Dr. Max Ezzati, a highly respected reproductive endocrinologist, to discuss the most important thing to know before starting IVF.

Dr. Ezzati shares key insights about how patients can optimize their chances of success, the factors that influence embryo quality, and why understanding ovarian reserve is crucial. We also dive into the role of lifestyle factors, genetic screening, and cutting-edge advancements in reproductive medicine. If you're considering IVF or supporting someone on their fertility journey, this conversation will empower you with the knowledge you need.

One important concept we discuss is the IVF pyramid, which illustrates how the number of eggs retrieved translates to the number of viable embryos. Not every egg leads to a healthy embryo, and understanding this process can help patients set realistic expectations about their IVF journey.

    In this episode, we cover:

How to assess your ovarian reserve before IVF
Why embryo quality matters and how it's evaluated
The importance of individualized treatment plans
How lifestyle factors, such as diet and stress, impact IVF outcomes
The role of genetic screening in IVF success
Common misconceptions about IVF and what patients should really focus on
Read the full show notes on my website.

I'm 40. Did 5 IVF Cycles, Got Dozens of Eggs, But Not a Single Normal Embryo. What Should I Do Next? (Ask The Egg Whisperer with guest Dr. Steven Palter)

    2026-03-18 | 30 mins.
Welcome back to Ask the Egg Whisperer! I'm so excited to have Dr. Steven Palter join me on today's episode. Dr. Palter is the medical and scientific director at Gold Coast IVF in Woodbury, New York, and an internationally recognized leader in fertility treatment. He's also the creator of the PCOS to Pregnancy Protocol -  the first comprehensive remote system targeting the root causes of PCOS and the Palter Fertility Method, a personalized approach to diagnosis and care that integrates metabolic health, surgical precision, and emerging reproductive technologies.
Watch the full conversation (with more live chatted questions) on Dr. Aimee's Instagram Live.
In this episode, we tackle real questions from our community (women ranging from 36 to 47), dealing with everything from diminished ovarian reserve and repeated IVF failure to PCOS, endometriosis, and experimental treatments like mitochondrial replacement therapy. Dr. Palter and I don't always agree, and that's what makes this conversation so valuable. We get into the nuances of DHEA and testosterone supplementation, when genetic testing is worth it even with limited embryos, why your stimulation protocol might be the problem, and how targeting insulin resistance can transform outcomes for PCOS patients.
    In this episode, we cover:
Why AMH predicts egg quantity, not quality  and why that distinction matters
The truth about DHEA and testosterone priming, including a major new study showing no benefit
Mitochondrial replacement therapy (three-parent IVF): promise, limitations, and what the data actually shows
Fresh vs. frozen transfers with diminished ovarian reserve, and why PGTA testing is still valuable
The #1 reason for immature eggs and no blasts and the protocol fix that works 80% of the time
Endometriosis, BCL6 testing, and when suppression vs. surgery makes sense
How targeting insulin resistance and inflammation in PCOS patients is changing outcomes dramatically
